First Weekend Waterfowl Hunting Application for
Controlled Hunts on Tule Lake & Lower Klamath Refuges, California

In order to hunt Tule Lake and Lower Klamath (CA side) Refuges during the first two days of the hunt season, you must apply and be successfully drawn. Each year approximately 3000 hunters apply for the limited number of openings.

Important Information

• Enclose a non-refundable application fee of $3.00 for each hunter listed on this application.
• Incomplete and illegible applications will be rejected.
• MAKE SURE ALL ADDRESSES ARE CORRECT...each hunter will be notified individually.
• Hunters may be named on one application ONLY and may NOT apply for the Klamath Basin Refuge Hunts if they also apply for the first weekend hunts on Modoc Refuge.
• ALL APPLICANTS will be notified individually by September 10 if successful and must return permit fees by September 25, 2000.
• Permit fees of $10.00 per hunter ($5.00 per Golden Age or Golden Access hunter) will be charged successful applicants. Season or multi-day passes may be purchased in lieu of paying permit and daily spaced-blind fees.
• Permits and Golden Age/Access Passports must be carried in the field by successful applicants.
• Junior hunters (15 years of age and under) will not be charged a fee, but must be accompanied by a permit holder over 18 and carry a Junior Hunter license while hunting.
• To qualify for Golden Age or Golden Access discounts, hunters must provide evidence that they possess these cards. No refunds will be given to hunters who fail to obtain these documents prior to applying for the first weekend hunt.

Instructions

• List your party's first (1) and second (2) preference for first weekend hunts - Lower Klamath, Tule Lake Field, Tule Lake Marsh.
• List the name, CORRECT MAILING ADDRESS and telephone number  for each member (four maximum) of your party below.
• If members have Golden Age or Golden Access Passports, list the passport number in the space provided to qualify for reduced permit fee. No refunds will be made if this information is not provided at the time of application. Leave blank if not applicable.
• Up to two junior hunters may accompany parties with permits. Do NOT  list Junior hunters on this application.
• All applicants will be individually notified by mail if the group is successfully drawn for this hunt.

• A non-refundable application fee of $3.00 for each hunter listed must be enclosed with this application. You may submit a single check for all members of your group. Make check or money order payable to U.S. Fish and Wildlife Service. Do not send cash.

  This application must be postmarked between August 1 and August 15, 2002 to be considered.
